For anyone that's confused; this man did not disappear for 6 years. That first ep was them meeting in the past. This couple dated for 5 to 6 years before Nowaki left to do his study abroad (which was revealed in the next ep that it was only for 1 year). And he did tell Hiroki about it, he just wasn't paying attention when Nowaki mentioned it.
This man is actually one of the best and purest characters in the anime. He really loves Hiroki and does his best to understand him and his needs.
Well I don't about other seasons but I have watched season 1 Hiro-san was informed by him that he's going abroad for study but Hiro-san always busy with his own work that he didn't paid attention to it and moreover he's indeed possessive toward his lover.... i really like This Doctor-Sama

I love this character he's so beautiful, totally misunderstood, and he truly loves Hiroki. I wish he had more screen time to be honest.
