My tentative stance is that the main character can be called a trans man, though I don't think the author had that specific idea in mind. (Consider that this was published in the 80s.) What I believe she conceived was something along these lines: a woman with the heart of a man, who believes she was given a female body as a cruel joke by God. Consequently, they insist they are actually a man on many occasions, and the question of their gender is at the heart of the story. Said story is about a 'princess' under immense pressure to get married by everyone around them, particularly their father, the king. Their mother is dead, and they have no siblings. So if they do not marry, there will be no one to take the place of king after their father, and the kingdom may be lost to its semi-hostile neighbor. But the 'princess' refuses to marry a man, insists they are man themself, and begins a secret relationship with a servant girl whom they dream of making their wife. SPOILER The prophecy referenced in the title states that when a particular sword that each heir of the kingdom inherits is wielded by one who is both man and woman, the kingdom will be saved. But when it is taken by one who is neither man nor woman, the kingdom will be lost. The question of whether the main character is the former or latter is a major aspect of the plot.

YO wanted to recommend some more manga for the list! There's Fire Punch tho also that's Fire Punch so I would recommend people not reading it unless they're ok with like. Heavy subject matter (in big, bold, sparkley text cuz it gets real bad at points (especially in the beginning), big trigger warnings for Cannabilism, rape, beastiality, incest, along with a lot of gore, death, nudity (also idk if there's a way to spoiler on anime planet so I hope a strikethrough works LoL). I don't wanna spoiler so I'd just say read the whole manga LMAO It's. Really good...by the same guy who did Chainsaw Man if that mmeans anything to you LMAO
There's also Zenbu Kimi no Sei which at first glance is just an ecchi gender bender manga BUT I SWEAR IT'S GOOD it has a trans male AND a trans woman protagonist and though there is a lot of fan service it's cute in it's own way
There's also Sket Dance which features a minor character nearing the end of the story who is a trans man who refuses to speak because of voice dysphoria, u don't have to read all of sket dance to get to his part of the story (tbh I just skipped everything and read the pages that were about him...), he appears in chapters 280-285 (each of which is called last dance)

IIRC regarding #9 on the list Kyou Kara Yonshimai, the trans girl is currently dating another (cis) woman in the story. i dont think the heterosexual tag is appropriate in that instance (though there might be a het romance with the mc? i forget all the details)

This story follows a character invited to an "otokonoko" maid cafe where they cross dress. Upon being invited, Mogumo, the main character, states they are neither female or male. This is the only manga I've found with a non-binary character. Spoilery here for the first volume,
But one of the people who works there realises she is female, not just a crossdresser.
The owner of the cafe is a trans woman as well. Finished, licensed in English, with five volumes (28 chapters).
